FacultyTools is a three-in-one web-based educational data collection system. The application has three main interconnected modules, though users can subscribe to one or two modules to meet their needs. The modules are: Student Testing, On-line Surveys (for polling teacher, parents, students), Observation Forms (walk-through, coaching, SPED student evaluations). We have a number of K-12, college/university and industry users, mostly in the Western United States. Some key features:

The system is customizable. You can define tests, forms, surveys, and align most content to goals, learning objectives, etc.
Observation forms can be entered via Windows Mobile, Palm, and Apple iPhone/iPod Touch device; and tests answered via Palms.
Additionally, we have a Math/ELA item bank available for grades 2-8, and 1,250 high school items across multiple subjects.
(NEW) We have a new statistics/analytics module that can be used to set up calculations on data from one or more modules.
(NEW) Our new iPhone/iPod Touch clients let you can create forms and subject lists and collect observations via your Apple device.
(NEW) Our library has dozens of ready-to-use tests & forms that can be copied into your account, and altered to suit your needs.

With this account, you can use and evaluate the full system for a period of 3 months. If you have access to the Apple, Palm or Windows Mobile client, you'll be able to collect data after minimal set up.
